This is the revised version of the 5W form (version Jan 7), which we invite you to fill in on behalf of your organization. By providing this information, you contribute to improving the clarity and effectiveness of Shelter activities in the 2024 Typhoon and Flood Response.

The updated form can be found under the "5W" tab. The form includes data you have previously submitted. Kindly review and update this information as needed.

Key Updates in the Jan 7 Version:

• Apart from formatting updates no big changes compared to the Dec version.

Revised Structure:

This version is based on the original OCHA 5W form and now includes newly defined Sectoral Activities. Also, three additional columns have been added for more detailed reporting on activities. Newly added columns are highlighted in red for easy identification. However, not all activities require these columns to be completed—only when a dropdown option appears. Please refer to the example provided on the right.

Cash Activities:

All cash modalities, including cash top-ups for materials, are now reported separately from sectoral activities. Refer to the example on the right for guidance.

Rubble Removal and Debris Management:

As agreed during the Shelter Cluster Information Management meeting, debris removal should only be reported if it facilitates families returning to their homes or clearing yards. Other debris-related activities fall under CCCM.
